Thank you for your interest! We are currently NOT scheduling appointments for Moderna vaccines using this form. Instead, please call 202-687-2200, option 2, to reach the Student Health Center and schedule a Moderna vaccine appointment directly with them. If you have trouble doing so, please email hoyavaxa@georgetown.edu and the vaccine coordinator will assist you from there.
